COVID-19 Response
For the safety of clients and community if you are not up to date with your vaccination including booster I am unable to provide face to face therapy services. Up to date means a person has received all recommended COVID-19 vaccines, including any booster dose(s) when eligible. For information on vaccination status please visit this site.
If you are unsure of your symptoms, from an affected area or simply wish to minimise impact, please ask for a Telehealth (on-line) appointment.

Therapy Terms & Conditions
Your Safety
Your safety is paramount, we do our best to create a comfortable indoor environment. We comply and follow the policies as set out by NSW services. These are constantly evolving and we expect individuals to take responsibility for informing us of any health concerns.
Sessions conducted outdoors are only delivered in special circumstances and with full agreement and discretion of all parties involved. Privacy in an outdoor public setting can not be fully guaranteed. Weather may require alternative arrangements of the session being delivered by video conferencing or phone.

Confidentiality
All client information is kept strictly confidential and is for our internal use only. If we become aware of any concerns regarding child abuse; your involvement in any serious criminal acts or if we are concerned about serious harm to self or others we are then obliged by law to report to the appropriate authorities (eg. Medical doctors, police, etc). Be aware that in any matters regarding the court of law, the state may petition our client notes.
Psychiatric Care
If you are currently under the care of either: a psychologist; psychiatrist or medical doctor regarding any mental health issues or other medical concerns which may be linked to your current problem, you will need to present a letter of referral from them before we start therapy. This means a short written note stating that you have informed your health care provider of your decision to engage in alternative therapies and techniques. If you have been prescribed medications by a licensed person or other practitioner, we request that you provde us a list of all current prescribed or alternative medications being taken. Furthermore, nothing that happens here in therapy should be construed as, nor should you believe that it is a substitute for the advice of a licensed person.
Your Choice - Your Responsibility
While during the session, I may offer ideas, suggestions or other ways of looking at your challenges and solutions for your consideration, you hereby agree that whatever we discuss is only my perspective and is not binding upon you, nor is it a prescription. If you want to discuss our suggestions with someone else, you should discuss them with a licensed health care provider. It is your responsibility to confirm whether or not any changes we made produce the desired results. It is your responsibility to communicate your results to us. My liability is limited to the amount paid for the therapy.
Payment
I currently do not accept credit cards. Payment is preferred by Cash or Direct Deposit. Direct deposit payment is required at least 3 days prior to an appointment.

Late payment
Payments not received by the date of our scheduled appointment may incurr an admin fee of $25.
Cancellation of Appointment
If you are unable to attend any appointment for any reason, I respect that you will contact me either via phone or email at least 48 hours prior to your appointment allowing alternative arrangements to be made. Otherwise a cancelation fee of the full fee may be charged.
Business Terms & Conditions
Payment
All training fees must be paid no less than seven (7) business days prior to the commencement of training, unless otherwise agreed.
Octavian does not accept purchase orders.
Cancellation/Rescheduling by Participant
All cancellation notices must be made in writing. Octavian is not responsible for any error in the delivery of such notices.
A booking can be cancelled/rescheduled up to 7 days prior to training commencement. If the booking party cancels/reschedules after this time a 50% refund will be given.
Note: If training manuals have been requested by the participants and have been purchased by Octavian, no refund on training manuals can be given. If training has been cancelled, the participant organisation will organise collection/delivery at cost to them.
Cancellation by Octavian
Training can be cancelled without prior warning, and money refunded to the booking party if due to any unforeseen circumstances or causes beyond Octavian reasonable control, including, but not limited to, acts of God, war, riot, embargoes, acts of civil or military authorities, acts of terrorism or sabotage, electronic viruses, worms or corrupting microcode, delay in delivery by Octavian vendors, fire, flood, earthquake, accident, strikes, inability to secure transportation, facilities, fuel, energy, labour or materials. If a booking has to be cancelled/rescheduled for any other reason, Octavian will notify the booking party no less than seven (7) calendar days prior to start of training and provide full refund of course fees.
Warranty
Octavian warrants that its personnel are suitably qualified and experienced to perform Training. Except as expressly represented otherwise, and to the extent not prohibited by law, all training, including any documentation, publications, software programs or code, and other information provided by or on behalf of Octavian to customer are furnished on an "AS-IS" basis, without warranty of any kind, whether express, implied, statutory or otherwise especially as to quality, reliability, timeliness, usefulness, sufficiency and accuracy. All implied warranties, including without limitation all implied warranties of condition, merchantability and fitness for a particular purpose are disclaimed by hot chilli. No oral or written information provided by Octavian shall create a warranty unless incorporated into these terms.
Liabilities
To the extent not prohibited by law, Octavian will not be liable for any consequential, incidental, special or other indirect damages, such as lost profits, arising from these terms even if Octavian has knowledge of the likelihood of such damages. In the event that Octavian shall fail to provide training in accordance with these terms, Octavian entire liability and customer's exclusive remedy for breach of these terms shall be for Octavian to use its reasonable efforts to repeat the training within a reasonable period of time; provided, that in the event Octavian is unable to so repeat, Octavian may elect to refund all payments actually received by Octavian from customer for the training in question, in full satisfaction of Octavian obligations. Such refund shall constitute Octavian entire liability and customer's exclusive remedy for such breach. In no event shall the aggregate liability for damages of Octavian, its employees or agents, arising from these terms whether by contract or tort exceed the amounts customer actually paid Octavian. To the extent not prohibited by law, the limitations in this section shall apply to personal injury and death.
Indemnification
Customer agrees to indemnify and hold Octavian harmless against any and all claims resulting from the fault or negligence of Customer or its Attendees in connection with these Terms.
Onsite Facilities and Equipment
For Onsite Training, customer shall advise if facilities and equipment in accordance with Octavian requirements to perform the training are available. Customer shall be notified of Octavian’s requirements. At a minimum, the facilities shall be clean, safe and free of excessive noise.
Copyright
All Materials supplied in respect of the training is copyrighted and remains the intellectual property of Deborah Edwards and Octavian. Reproduction of this material is not permitted without authorisation.
I hereby acknowledge and accept the terms and conditions of Octavian as written above. I adhere now to the commencement of the training to be developed and delivered.
Supervision Terms & Conditions
CLINICAL HYPNOTHERAPY SUPERVISION AGREEMENT
Duties and Responsibilities of Supervisor and Supervisee:
For the Supervisor:
• To examine diagnoses, interventions, and treatment plans made by supervisee;
• To challenge supervisee to validate approaches and techniques used;
• To monitor basic micro-skills;
• To present and model appropriate behaviours and directives.
• To intervene where client welfare is at stake;
• To ensure ethical guidelines and professional standards are maintained; and
• To obtain appropriate consultation when necessary.

For the Supervisee:
• To uphold ethical guidelines and professional standards;
• To be prepared to discuss client cases with the aid of written case notes and / or video / audio tapes;
• To validate diagnoses, interventions, approaches and techniques used.
• To be open to change and use alternate methods of practice if required.
• To consult supervisor or designated contact person in cases of emergency.
• Implement supervisor directives in subsequent sessions.
• Maintain a commitment to on-going counsellor education and the counselling profession.
Structure of Supervision:
Individual and Group Supervision are available for your professional and personal development.
Group Supervision comprises of a minimum of 3 – 5 members. Prior to your attendance a phone interview and face to face induction session will occur to ensure the best fit for the group.
To ensure consistency and respect for all group members, we request a commitment to attend 3-6 consecutive sessions.
Individual Supervision Do not require a minimum commitment and are conducted as required.
Our agreed expectations:

Confidentiality: what happens during every supervision session must remain confidential. When discussing clients, be careful not to advise of any characteristics or information which may disclose your clients’ identity. Please refer to the document Presenting Clients during Supervision.

Be on time: please make a conscious effort to be on time for all sessions. Be aware that lateness may result in your being asked to leave group supervision and/or shorten the time period for your session.

Be conscious: Be here because you want to be here - uptime, listening, holding right intention, participating in the group and showing respect to each member at all times.
Your intention focused in the right place is stronger than simply having attention.

Act professionally: have integrity to take care of your physical, emotional & mental health. We expect you to have a respectful approach and attitude at all times.
Your business follows the Health Care Act as specified by the NSW government. All professional requirements as set out by the boards with which you belong are adhered to and maintained.

Procedural considerations:
a. Supervisee’s written cases notes (plus diagnoses and treatment plans) and audio / video tapes may be
reviewed in each session;
b. Issues relating to supervisee’s professional development will be discussed;
c. Sessions will be used to discuss issues of conflict and failure of either party to abide by the guidelines outlined in this contract. If concerns of either party are not resolved in supervision, A representative from the relevant professional association will be consulted;
d. In event of an emergency, supervisee to contact supervisor.

Clean boundaries maintain only appropriate contact between clients and yourself inside and outside of the therapy room. Do not entertain personal relationships with the clients. As per the codes of ethics, it is recommended that no personal relationships are made for at least 2 years after completing any therapy work together. There is no failure only feedback

Providing Feedback: We expect that we all participants to have the sensory acuity and look outside self, providing constructive feedback. To have an honest and openness to receive feedback with the flexibility to change or modify our behaviour and ways of thinking if necessary.

Responsibility & Accountability: Treat everyone as a Professional and as an adult. You are responsible and accountable for your own actions and behaviours. As a Professional and as an adult, we take it for granted that you will follow through with your any agreed actions &/or communicate if you need help or if you need to make any changes or alternative arrangements.

Location of Supervision:
Both Group and Individual supervision sessions are available at:
• In Person
• Phone & Video conferencing sessions are also available
• Or as arranged between supervisor and supervisee(s).
If an alternate location or change is required, you will be advised with a minimum of 3 days’ notice.
Cancellation Fee:
For Individual supervision 3 working days or less notification may incur a fee of up to $250
For Group supervision: 3 working days or less notification incurs a fee of $110.

Late Payment Fee: If full payment is not received before or on the day of supervision a late payment fee of $25 applies.
Revision of these terms and conditions may be made upon the request of either the supervisee or the supervisor.
A review of the supervision agreement may be conducted every six sessions or 6-month period.
It is the responsibility of individual members to review all changes.
